Introducing a new way for you to get the help you need

At Chapelford Medical Centre, we understand the challenges patients all over the country are faced with when accessing healthcare and we are constantly striving to offer innovative ways to enhance the service that we provide.

As of Wednesday 22nd February, we are proud to offer a new way to access services at the surgery and ensure you are able to get the help you need from the right person at the right time.

Our new Digital Triage Service will be available through our practice website.

On completing an online questionnaire, we will assess your health concerns and queries, and direct the information to our experienced team to ensure that you receive the best possible care, first time.

The Digital Triage service will be managed by a dedicated team of Care coordinators and Advanced Nurse Practitioners. The team will contact you via SMS to provide assurance that your query has been received and communicate when to expect follow up.

All staff have been specifically trained to triage incoming queries and ensure that you are directed to the staff who are best equipped to help you.

Via this assess symptoms, give advice, issue prescriptions or arrange appointments with clinicians, as well as complete routine administration work such as fit notes.

In some instances, the triage system will be able to provide immediate advice to ensure that there is no delay in you being directed to the best possible service, even during evenings and weekends.

Whilst the practice phone line will remain open during office hours, the new digital triage service will be available 24 hours a day, seven days a week, and we would encourage you to direct any non-urgent queries via this service.
